Help Card



All emergency rescue services (Emergency Rescue, Fire Brigade, Police, GOPR, TOPR, WOPR) usually called in case of accidents have been informed about the
“HelpCard”, and about stickers, emblems, stamps and information they contain.

The person who orders the card has to fill in the card order form and is fully responsible for filling in the appropriate details.
The time between placing an order and receipt of the ordered card, stickers, emblems and stamps is 7 working days from the moment the order is received.
After sending the “HelpCard” to the ordering person, the company will retain the information contained in the card for 14 days in case of complaints, if an error occurs on its part. After the time elapses, only the details containing name, surname and PERSONAL ID NUMBER will be stored.
All remaining information, i.e. a photo, phone number, blood group, the ailment and declaration will be removed.
The data submitted by you are protected (Official Bulletin no.133, item 883 of 29 August 1997, revised) and will not be processed or made available to anyone.
We hold a permit from the General Inspector of Personal Information Protection for collecting and storing personal information.

In case the card is lost, damaged, stolen or the information in it must be changed, the person who wants to buy a new card should phone our company at:
+48 608 711 991 to apply for discount.


The first page of "HelpCard" contains necessary card owner personal information, such as:

  • nationality (national flag + letters standing for the name of the country)
  • Foreign Affairs Ministry phone number (in native country of the card owner) - very important, when we are abroad and an accident occurs,
  • a photo
  • surname
  • name
  • PERSONAL ID NUMBER
  • phone number of the person to be informed in case of accident,
  • blood group
  • declaration of will [YES or NO] which allows to donate body organs after the card owner`s death card owner`s ailments and medical condition, essential for emergency rescue services at the time of rescue action:

Page two of "HELP CARD" informs about emergency telephone numbers:

  • 112 all emergency calls
  • And first aid tips, so important in the first minutes after the accident.
